Closing Care Gaps Before Year-End: A Health Plan's Q4 Playbook

Q4 changes the math. The care gaps still open at this point in the measurement year are not the easy ones. They are the members who did not respond to the January outreach, did not book after the spring reminder, and have not engaged with a single communication since open enrollment. Chasing them the same way you chased everyone else will not close the gap in time.

This is not an argument for why engagement matters. You already know that. This is a working playbook for what to do with the time you have left.

Why Q4 Gaps Are Different

The gaps that survive to Q4 share a pattern. They are concentrated among members who are hardest to reach: outdated contact information, low historical engagement, chronic no-shows on preventive visits. A generic mailer or a call campaign was already tried and failed with this group once this year. Running the same play again mostly burns time you do not have.

The fix is not more volume. It is a shift from broad outreach to targeted, engagement-led outreach that meets members where they actually respond.

Step 1: Segment by Behavior, Not Just by Measure

Most gap-closure lists are built around clinical criteria: which members are missing which HEDIS measure. That is necessary, but it is not sufficient in Q4. Layer a behavioral view on top of it. Which of these members have engaged with any communication in the last 90 days? Which have never opened an email or clicked a reminder?

Members who are already engaged in some channel are the fastest path to closing a gap before year-end. Members who have gone dark need a different approach entirely, often a change in channel or messenger rather than another repeat of the same ask. Prioritizing by behavior, not just by measure, tells you where your remaining Q4 effort will actually convert.

Step 2: Time Outreach to When Members Actually Act

Timing is a lever most plans underuse in Q4. A preventive screening reminder sent the week of a member's birthday or tied to a seasonal moment (flu season, back-to-school checkups, year-end benefit deadlines) performs differently than the same message sent in isolation. Curated, condition-based reminders tied to a calendar moment give members a reason to act now instead of filing the message away for later.

This matters more in Q4 than any other quarter. Members are already fielding open enrollment communications, year-end benefit reminders, and holiday noise. A reminder that lands at a relevant moment cuts through. One that lands generically gets buried.

Step 3: Make the Next Step Frictionless

This is where most gap-closure campaigns lose members, not in the reminder itself, but in what happens after. If closing the loop requires a phone call during business hours, a portal login the member has never used, or a form buried three clicks deep, the gap stays open even when the member intended to act.

The plans seeing the fastest Q4 movement are the ones that shrink the distance between reminder and action. A direct link to scheduling. A one-tap confirmation. No new login to remember. Cix Health clients running engagement-first outreach see a 71% member interaction rate, compared to a 17% industry average, and the difference consistently traces back to friction. Members act when acting is easy. They do not when it is not.

Step 4: Track Engagement, Not Just Outreach Volume

It is tempting to report on volume this time of year: number of reminders sent, calls placed, letters mailed. Volume is not the leading indicator of whether a gap closes. Interaction is.

Track opens, clicks, and completed next steps, not just sends. A campaign that reaches 10,000 members but generates almost no interaction is not on track to close gaps, no matter how the send numbers look on a dashboard. A smaller campaign with strong interaction is doing the actual work. Reporting on engagement gives you a real-time read on whether your Q4 push will land before the measurement year closes, while there is still time to adjust.

What This Sets Up for Next Year

Closing this year's gaps matters on its own. But the infrastructure built to do it, behavioral segmentation, timely and relevant reminders, low-friction next steps, does not expire when the measurement year ends. It is the same infrastructure that makes next year's HEDIS cycle start from a stronger position instead of the same scramble.

Q4 outreach done well is not just damage control. It is the beginning of a better Star Ratings cycle for the year ahead.
